Student Forms

These forms are for use by students: the Pre-Approval Form for Employment Eligibility is for both graduate and undergraduate students; the Minor Form and Department Honors Application Form are for undergraduate students, and the Departmental Intent to Graduate Form is for graduate students.

Undergraduate Application for Degree

This form is due before the end of the semester prior to that in which you are graduating. However, to help insure your degree requirements are met, it is strongly recommended that you submit this for by the following dates (over a year prior to graduation):

April 15 for graduation for the following spring semester

November 15 for graduation for the following fall semester

July 15 for graduation for the following summer semester

Students need to submit transcripts and lobotrax degree audit along with the form to Nicole Bigham, Senior Academic Advisor, nicluna@unm.edu, CENT 3012.

Pre-Approval Form for Employment Eligibility

To begin the process of securing employment in the department, either through an assistantship for graduate students (RA, PA, TA, or GA), or through various student employment positions (graduate and undergraduate students), you must complete this form and return it to the department.

If you have another employment position outside the department, you must take this form to your supervisor in that position and have him/her certify the information by signing that section of the form. 

Please be clear on UNM Policy regarding employment.
Students are not authorized to begin work until they have either:

  1. been given clearance from OGS via email (graduate students) OR
  2. been given clearance from Student Employment (either undergraduate or graduate students) that you are cleared to work.

You will absolutely NOT be paid for any hours that you have worked PRIOR to your authorized start date!

Minor Form

Some departments at UNM allow an undergraduate student to minor in that subject. A minor requires a specific number of credit hours and perhaps specific coursework and grade point average. You can find the allowed minors by checking the online UNM catalog or an individual department’s web site.

If a CCEE undergraduate student wishes to declare a minor, you must print this form, complete the top section, then take it to the Advisor in the department in which you want the Minor. That Advisor will fill in the courses needed and sign the form. You must also sign the form. Then return the form to the Senior Academic Advisor in CCEE. She will keep the form in your file. When you apply for graduation, she will check to make sure that you have completed all the minor requirements and will certify to the university that you have the minor so that it will be included on your academic transcript.

CCEE Department Honors Application

Undergraduate students in the junior year who are exceptionally motivated to study a topic in more detail and/or work with a particular faculty member may be eligible to apply for the Departmental Honors Program. More detail about the Departmental Honors Program can be found under Students, Undergraduate.

To apply, you must meet the eligibility requirements and have an agreement to do independent study with one of our faculty members. Print and complete the form, including the signature of the faculty member with whom you will be working and the Chair of the department. Then return the form to the . She will keep the form in your file. When you apply for graduation, she will check to make sure that you have met the requirements and will certify to the university that you should be awarded the appropriate departmental honors.

Departmental Intent to Graduate

In order for the department to list a student on the official OGS graduation list, graduate students must complete a Departmental Intent to Graduate form prior to the semester in which they intend to graduate.

Print the form and complete it, including your signature. Give the form to the Senior Academic Advisor by these deadlines: July 20 for fall graduation, December 5 for spring graduation, or May 2 for summer graduation. You must have your Program of Studies or Application for Candidacy already approved by the Dean of Graduate Studies before the department can accept this form from you. This form allows the Senior Academic Ador to include you on the official OGS graduation list for that semester.

Minor in Construction Management

The minor in construction management requires 18 credit hours.  Some courses are offered online only. 

Use this form to apply, then take it to the CCEE Senior Academic Advisor, Nicole Bingham, nicluna@unm.edu, 277-6633.  Once you have signed it and she approves it, take it to your advisor in your department for his/her signature. Your major advisor will certify that you have completed the requirements when you are ready for graduation.
